Guardian Shrine is a Random Event in Remnant: From the Ashes. The Guardian Shrine event can appear at Yaesha. Random events act like side quests where players need to complete some requests in order to obtain a reward. The events may or may not appear on your play through, requiring a restart for a chance of getting them.

Guardian Shrine Description
- The Guardian Shrine is an event dungeon that appears on Yaesha. After entering the dungeon, you will be met by a giant stone door, which holds a Tome of Knowledge and the helmet, Radiant Visage. Acquiring the Strange Curio which can be found at the back Stuck Merchant cart will allow you to unlock this door, but be warned: you will immediately be set upon by the Root Horror after picking up both items.

boss disappears and attacks merchant in the yaesha forest, that's where you continue the Root Horror fight.

Dude no matter how many times i restart my game it never appears, what an annoyance.

Make sure you kill the boss, if you die you can't redo the quest until reroll of campaign
This event being a random spawn and not paired with Stuck Merchant (strange curio) is such an annoyingly dumb waste of time for rng re-rolls.
